Program Overview
HSNY’s Community Health Programs (CHP) support the mission to end homelessness by addressing social determinants of health and food insecurity that place individuals and families at risk of housing instability. Food services are paired with nutrition and health education, case management, and linkages to essential resources.
Nearly one in nine New Yorkers statewide struggles to reliably access nutritious food. CHP responds directly, ensuring community members have the resources they need to regain stability and keep it.
Programs Within CHP

Food and Nutrition Services (Ryan White Part A): client-choice pantry, nutrition counseling, and cooking classes for people living with HIV

Real Connections (Ryan White Part B): non-medical case management, food vouchers, health classes, and support groups

CHP Community Food Pantry: supermarket-style, client-choice pantry, by appointment via the Plentiful app

Emergency Pantry Services: culturally diverse pantry kits for families in HSNY shelters
The No-Line Community Food Pantry
Our second No-Line pantry is now open in the Bronx. Designed like a grocery store, it lets clients choose foods that fit their needs and cultural traditions, with a registered dietitian on hand. No lines. No barriers. Just food, choice, and dignity.
Impact and Benefits
Access to nutritious food and health education supports physical and mental well-being while easing financial strain. By addressing food access and the social determinants of health, CHP helps prevent homelessness, supports recovery and independence, and strengthens community resilience.
Referrals and eligibility
Eligibility varies by program and funding source. Referrals may come through healthcare providers, community partners, HSNY programs, or self-referral where applicable.
