Connections to Care: Building Resilience in Youth (C2C:BRY), community-based mental health support in the Bronx.

Program Overview
C2C:BRY addresses the growing mental health needs of young people in the Bronx by integrating mental health support into trusted community-based settings, as part of the broader Connections to Care initiative.
According to Montefiore Medical Center, one in five youth in the Bronx experiences mental health challenges, often compounded by poverty, food and housing insecurity, and stigma around care. By meeting youth where they are, C2C:BRY reduces barriers and builds resilience before crises escalate.

How It Works
C2C:BRY delivers non-clinical mental health support through partner community-based organizations using an evidence-based task-shifting model: CBO staff receive training and coaching from mental health providers, deliver early identification and non-medical support, and refer youth to higher levels of care when needed. Services are integrated into existing youth-facing programs, in schools, family shelters, and partner sites like BronxWorks, to reduce stigma.
Impact and Benefits
Youth build resilience and coping skills and access support in environments they trust. Early support reduces barriers to care, mitigates the effects of trauma, and helps prevent school disengagement, homelessness, and involvement with emergency systems.
Referrals and eligibility
Youth may be referred through partner community-based organizations, schools, HSNY programs, or family shelters. Eligibility is based on age, residency, and program participation criteria.
