Program Overview

C.R.E.A.T.E. provides permanent, affordable housing paired with hands-on support for individuals who need ongoing assistance to remain stable and independent. Clients live in their own apartments while receiving individualized support from dedicated case managers.

Participation is voluntary and rooted in choice, respect, and person-centered care. The program uses a scattered-site model, with apartments located throughout the community rather than a single building, supporting real community integration.

Who We Serve

C.R.E.A.T.E. serves individuals and families who have experienced homelessness and may be living with mental health challenges, substance use disorders, HIV/AIDS, or other medical vulnerabilities, prioritizing dignity, autonomy, and individualized pathways to stability.

Services and Support

Case management and ongoing tenant support, typically weekly

Housing retention and eviction prevention

Crisis intervention, counseling, and peer support

Financial literacy, budgeting, and money management

Independent living and housekeeping skills development

Coordination with medical, mental health, and substance use providers

Impact and Benefits

Referrals and eligibility

Referrals come through city agencies, healthcare providers, community partners, and HSNY programs. Eligibility is determined by housing need, program criteria, and funding requirements.
