
At Housing Solutions of New York, homelessness is not a single moment. It is a journey, and so is the path out of it.
Our programs are designed to support individuals, families, and youth at every stage of that journey, from immediate shelter and basic needs to long-term housing stability and independence. Rather than offering simple shelter and isolated services, HSNY provides a coordinated continuum of care that meets people where they are and evolves as their needs change.
Clients may first enter HSNY through temporary or transitional housing, where safety and stability are prioritized. From there, they can access food and nutrition support, physical and behavioral health services, youth and family programming, workforce training and opportunities, in addition to pathways to supportive and permanent housing. Each program works hand in hand with the others, creating a holistic support system that treats the individual not just their housing status, but the whole person to address the underlying barriers causing their instability.
This integrated approach allows HSNY to respond to the complex realities of homelessness while keeping the ultimate goal in focus: helping people move from crisis to stability and remain housed for the long term.

In partnership with the New York City Department of Homeless Services (DHS), Housing Solutions of New York operates 12 Tier II Transitional Housing facilities and 6 commercial hotel sites across four boroughs. HSNY’s Temporary Transitional Housing program provides safe, stable housing for individuals and families experiencing homelessness as they work toward permanent housing solutions.

HSNY’s Community Health Programs supports the organization’s broader mission to end homelessness by addressing social determinants of health and food insecurities that place individuals and families at risk of housing instability. By pairing food services with nutrition and health education, case-management, and linkages to essential resources, this program helps stabilize individuals and communities while supporting their journey to permanent housing.

Community, Resilience, Empowerment, Access, Transformation, and Equity (C.R.E.A.T.E.) Supportive Housing at Housing Solutions of New York provides permanent, affordable housing paired with hands-on support for individuals who need ongoing assistance to remain stable and independent. The values that C.R.E.A.T.E. stand for guide every aspect of the program.

The Health Home Care Management Program serves adults experiencing homelessness or housing instability living with chronic physical and serious mental health conditions such as HIV/AIDS, diabetes, hypertension, bipolar disorder and anxiety disorders.

HSNY’s Youth Services Program, Connections to Care: Building Resilience in Youth (C2C:BRY), addresses the growing mental health needs of young people in the Bronx by integrating mental health support into trusted community-based settings. The program is part of the broader Connections to Care initiative, which strengthens community-based mental health support for low-income New Yorkers.

HSNY’s Emergency Housing Program (EHP) works in partnership with the New York City Human Resources Administration (HRA) to ensure that individuals and families living in emergency housing receive safe, dignified, and compliant shelter while working toward permanent housing solutions.

The JUST Health (Jobs Undeniably Secure and Transcend) Employment Initiative is a comprehensive workforce pathway designed to assist people living with HIV in gaining meaningful employment and long-term stability. Grounded in justice and equity, the program recognizes that employment is directly related to our clients’ health and housing status.

Works Plus is a workforce readiness initiative designed to support youth and young adults affected by the Justice System as they prepare for employment and long-term independence. The program meets clients where they are, offering flexible scheduling and individualized support that reflects each person’s needs, goals, and availability.
